The Maryville Site Request Form: what it asks for and how to avoid a send-back

One request, two names

Maryville help articles from the eValue years, 2021 to 2023, call it the Site Request Form, and a 2023 article says an “eValue Processing Team” reviewed those requests. Maryville's March 2025 approvals article describes the same step in Exxat: open the Coursework icon, go to the course wishlist, and follow the request through View Details.

Under either name the request does one job. It is how you get official Exxat approval of the site and preceptor request, the second of the four approvals Maryville requires before clinical starts.

Maryville's 2026 preceptor blog says its online resource center, updated each semester, covers how to submit site requests. In practice that is the Graduate Nursing Clinical Resource Center, which holds the current instructions. What goes into the request

Maryville's public pages don't list the form's fields. What Maryville does describe is the material a request carries.

• The course it is for, since Exxat files each request under that course's wishlist
• The clinical site, which Maryville contracts with through its MOU (Memorandum of Understanding) or through the site's own agreement
• The preceptor, who has to meet Maryville's preceptor rules
• The preceptor's state license and board certification, which a 2021 help article says students look up online and upload
• An approved Preceptor Exception Form, uploaded to “My Request”, if the preceptor is not board certified

Three things no public Maryville source mentions: a named preceptor agreement, a required preceptor CV, or a signature page for the request. If your course asks for any of them, the instruction will be in your clinical packet or the Resource Center, not on Maryville's public site.

Check the preceptor before you enter them

Most of what a reviewer checks about a preceptor comes from rules Maryville publishes. Its February 2026 preceptor blog lists the minimum: the preceptor must be an APRN, MD or DO, hold board certification, be licensed without encumbrance in the state where they practice, and have a year or more of experience.

Relationships matter as much as credentials. The same post bars relatives, anyone who supervises you or reports to you, your own healthcare provider, and anyone you work beside directly or who evaluates your work. For PMHNP students, social workers with an LCSW and psychologists with a PsychD or PhD also qualify. Every rule, including the PMHNP detail, is set out on preceptor requirements.

For the uploads, a 2021 Maryville article points to Nursys for NP preceptors, noting that “a CNP license” can cover both the state license and board certification. It names separate lookup sites for physicians' licenses and board certification. A preceptor without board certification needs the Preceptor Exception Form approved by the Program Coordinator.

Match the site to your track

A qualified preceptor in the wrong setting still doesn't fit the course. In a 2023 webinar, Maryville described the setting each NP track needs. Each track also has three practicum courses, and the request is made for one of them.

Why a request comes back

Maryville doesn't publish a list of reasons it returns requests. Its own rules point to the likely ones, and each is easier to catch before you submit than after.

• The preceptor is a PA or CRNA, or otherwise outside the APRN, MD or DO rule
• No board certification, and no approved Preceptor Exception Form in “My Request”
• The preceptor's license is encumbered, or isn't held in the state where they practice
• A relationship Maryville excludes: relative, supervisor, direct report, direct work colleague or your own provider
• A setting that doesn't match the track, such as a hospitalist service for an FNP course
• An employer arrangement where you would be on the clock during clinical
• License or board-certification documents missing from the upload

Check View Details regularly once the request is in. Maryville's approvals article says that is where the status and comments appear, and a comment you answer the same day costs less time than one you find a week later.

Timing and review

Maryville doesn't publish current request deadlines or review times. The last public description is a 2023 article from the eValue years: requests were processed in the order received, the busiest stretch ran from a “priority request deadline” to the start of the semester, and a request submitted before that deadline was dealt with before the semester began. The article did not give the deadline dates themselves.

Whether Exxat review works the same way isn't public. If it does, a request entered early meets a shorter queue. Approval can lead to two more steps. Some sites run their own onboarding, which Maryville says doesn't replace its approval, and Maryville says to ask a site about a Maryville photo ID only after final Exxat approval.
