Maryville's Physical Examination Form, TB testing, and what follows a positive test

Two items you can handle in one visit

The physical and the TB test are separate items in your Exxat Compliance module, but they are easy to plan together. Maryville's 2026–27 catalog lists a physical examination and TB testing among the requirements you must meet before required clinicals, next to immunizations, BLS and screening. The whole list is on clinical compliance.

The physical is Maryville's own form, not a letter from your doctor. The TB test has more than one route, and the route you pick changes how many visits it takes. If a TB result is positive, a third document comes into play: Maryville's Symptom Free Form.

Both have to hold for the whole time you are in clinical. Maryville's June 2026 article on attending with missing requirements says a lapsed item puts your Exxat account on hold, and hours logged while out of compliance must be repeated.

The Physical Examination Form

Maryville's Ask Maryville article “Where do I find the Physical Form” (February 2026) says the form is a 3-page template inside the Exxat Compliance module. An MD, DO, NP or PA has to complete it, and all 3 pages have to be uploaded.

Download the template before you book the appointment and take it with you. Maryville's article points to its own form, so don't rely on a visit summary or your clinic's standard physical paperwork to stand in for it. How to find your way around the platform is on our Exxat page.

Missing pages are the easy mistake. Before you leave the office, check that the clinician has completed every page, then scan all 3 in order. A form with a page missing is likely to be sent back, and you lose the days it takes to resubmit.

Maryville's public pages don't say whether the physical has to be repeated each year. Check the item's status in Exxat, and ask approve@exxat.com if it isn't clear.

Your first TB test: skin test or blood test

Maryville's catalog asks for a 2-step PPD skin test at first, then yearly testing. The June 2026 Ask Maryville article on rejected TB tests adds that a QuantiFERON Gold or T-Spot blood test is accepted in place of the 2-step skin test.

Whichever route you take, the result has to be dated within 12 months. If you had a 2-step PPD or a blood test in the past year for work, check the date against that window before you book a new one.

Keeping the TB test inside the 12-month window

Maryville's article gives two timing rules: the test must be within 12 months, and within 12 months of your clinical start. Read together, a test that is current today can still be rejected if it will be more than a year old on the first day of your rotation.

The end date matters as much as the start date. Compliance has to stay current throughout clinical, so a TB test done 11 months before your rotation begins will lapse partway through it. The account hold that follows stops you attending and logging hours until a new result is approved.

Renew on your own calendar rather than waiting for a reminder. Note the anniversary of your last test, book the next one several weeks before it, and upload the result as soon as you have it. Sites can set their own window too: SSM Health's student policy asks for a TB test within the past year, and other systems are on St. Louis and Kansas City clinical sites.

After a positive TB result: chest X-ray and Maryville's Symptom Free Form

A positive skin or blood test changes the paperwork. Maryville's June 2026 article says a positive result requires a chest X-ray plus Maryville's Symptom Free Form, a document stating that you have no symptoms of TB. Plan for both well before your clinical start, since an X-ray means another appointment.

The Symptom Free Form is a Maryville document, so use Maryville's current version rather than a symptom questionnaire from an employer or clinic. Maryville's public pages don't say where the form is posted. Look first at the TB item in Exxat Compliance, then in the Graduate Nursing Clinical Resource Center, which Maryville says covers compliance and special paperwork.

Two more things aren't public: how often the chest X-ray has to be repeated after a positive result, and whether the symptom form is renewed each year. Ask the Online Clinical Team before your next renewal comes due. Ask each site as well, because a hospital may apply its own rules to students with a positive history.
